Free sports physicals available to Edmonds School District students on May 28

efcEdmonds Family Medicine providers and nursing staff are donating sports physical exams for Edmonds School District students on May 28.

Any District students entering grades 7-12 this fall, who have a sports exam expiring in the next 12 months, are welcome to take advantage of this offer. Sports physicals are good for two years from the date of the physical.

Students are encourage to make an appointment with Denise Lewis, Edmonds-Woodway High School Athletic Secretary, at: lewisde@edmonds.wednet.edu.

Appointments will not be confirmed until the office receives the parent consent form. Walk-ins will also be welcome on the day of the event with signed consent forms.

Edmonds Family Medicine is asking for a $20 suggested donation which will be donated to the Edmonds-Woodway sports medicine program. Checks can be made payable to ESD, Edmonds School District.

The event is May 28 from 2-6 p.m.at Edmonds Woodway High School, 7600 212th St. S.W., Edmonds.

Forms needed are the Sport Physical Form and the EFM Consent Form.
